Notice to Employers and Payroll Companies

A   separate "Application for Submission of Unclaimed Coogan Funds” form must be completed for each beneficiary and for each employer.   The information requested on this form will help to assure that the process of claiming funds is both effective and efficient.

All applicable questions should be answered as fully and accurately as possible. For example, there may not be a payroll company, and/or you may not have information on the beneficiary's agent. Checks should be made payable to The Actors Fund.
